This form allows you to request a site inspection for the items you select below. In order to use this form you must have information for all the "required" items shown below, marked with "*". Each permit number is site specific so please exercise care in specifying the inspection(s) for each separate permit number.

Inspection requests for incorrect site addresses, or where the work is not ready for inspection, will be subject to re-inspection charges/trip fees. Depending on work volume we cannot guarantee inspections on the dates you request. Do not use this form to request final inspections -- for final inspections, call the Permit Application Center at 503-588-6256, option 3, then option 1.

Please note that you must indicate "Community Development Department" in the bottom-left drop-down box IF it is asked for on the request form prior to submission.  To make a checkbox selection - the checkbox to the right of the inspection is the appropriate box for each.  Call 503-588-6256, option 3, then option 1 to report any submission issues or to get assistance.
