Automated External Defibrillator Registration

Your AED is a valuable investment and asset to the Salem community. The Salem Fire Department would like to add your AED to our list of known AEDs in the community.

SFD Fire Report Request Form

Form to request a copy of the report for a fire in the Salem Fire Department's jurisdiction. Fee is $10 per report.

Salem Fire Self Inspection Form

The self inspection form provides a tool to inspect your business for possible fire safety issues. Those pariticipating in the Self Inspection Program use this form to complete their inspections.

Medical Emergency Card

You can help speed treatment of you and your family members by keeping a medical emergency card for each family member. This card lists your medical history, current medications and known allergies. Additionally, it lists contacts in case of emergency, such as your primary physician.

Environmental Research Request Form

To request information from our records about any hazardous or environmental conditions that may exist or have existed on a particular property, download the Environmental Research Request form at left. Send the completed form to Fire Station 1 at the address listed on the form. You can also deliver your completed form in person to Fire Station 1. Fee is $35.

Salem Fire T-Shirt & Video Order Form

To order Salem Fire Department Supporter T-Shirts or Fire Safety Videos in both English and Spanish, complete this order form and return it to Fire Station 1.

Capital FireMed Brochure and Application

For an annual $50 membership all household members receive unlimited emergency ambulance service and most non-emergency ambulance service within the Capital FireMed service area. See the Emergency Medical Services Page for more information.

Salem Fire Model Rocket Launch Permit

Complete this permit and send it to the Salem Fire Department. There is no cost for this permit but it is required to be reviewed by the Salem Fire Department prior to any model rocket launch within the City of Salem.
