Coverage is voluntary and can be extended to include your qualified family members; ie spouse and child/ren.

Enrollment can be done at any time and premiums can be taken out of your pay check on a pre-tax basis.

• Increments of $25,000
• Minimum Amount $25,000
• Maximum Amount $300,000

All eligible persons may select any $25,000 increment up to $300,000.  Subject to 10 times your annual salary for amounts in excess of $150,000, on the date of your enrollment.
