FMLA/OFLA secures the rights of employees to respond to the critical health needs for yourself and any qualified family member and to guarantee reinstatement to their former/equivalent position.

To be eligible you must have worked at least:

  • FMLA - 1260 hours in the preceding 12 months
  • OFLA - 25 hours per week in the preceding 180 days

Reasons to apply:

  • Serious health condition or injury to self
  • Serious health condition or injury to your spouse / same sex domestic partner
  • Serious health condition or injury to your parent / parent-in-law
  • Serious health condition or injury to your grandparents / grandparents-in-law
  • Serious health condition or injury to your child / step-child
  • Birth of a child / adoption
  • Sick Child (OFLA)
  • Military Caregiver Leave
  • Military Exigency Leave

Protected time available

  • 12 weeks in a rolling 12 month period or
  • 480 intermittent hours in a rolling 12 month period

 

Accrued sick leave must be used first, then you can select any other accrued leave.

City of Salem will continue your medical/dental/vision/group life while you are on approved protected leave.

OFLA Sick Child Guidelines:

  • Routine medical and dental appointments, school closures and/or absent care provider do not qualify for ''Sick Child Leave". Please follow normal leave procedures.
  • Sick Child Leave is counted as OFLA only and does not deplete any allowance you may be entitled to under FMLA.
  • Sick Child Leave is to care for a child under age 18 for a non-serious illness. If the illness will require over 3 days off from work or is a recurring chronic health condition, this may qualify as a FMLA/OFLA Serious Health condition of a family member. Please complete a FMLA/OFLA Leave Request form for those situations.
