- You may download the application on-line by visiting www.travel.state.gov. Please make sure to print all pages of the application or you may obtain an application from our office at 340 Chemeketa St NE, Salem, Or 97301.

| 2. Proof of Citizenship: |
- A previously issued Passport OR
- A Certificate of Naturalization OR
- State Certified Birth Certificate
- Hospital Records are NOT acceptable
- If a birth certificate is being obtained from the State of California, the LONG FORM will be required, an abstract certificate will not be accepted
Note: Photocopies will not be accepted. Your original documents will be returned with your passport. Children under 14yrs of age, will need to provide a birth certificate showing both parent's names.

| 3.Two Identical Passport Photographs |
- Pictures taken within the last 6 months
- Photo size: 2"x2" taken against a plain white or off white background
Children under 18 can use parent's ID
- Valid driver's license. OR
- Other acceptable photo identification, refer to the reverse side of the passport application.

If you need your passport sooner than the normal 4-6 weeks, you may expedite your application for an additional $60.00 per application fee. The fee does not include urgent mailing or delivery service. This service must be arranged by the applicant. |

Currently, it is taking up to 1 week for Expedited Applications and up to 2 weeks for Routine Applications to be tracked online. Please allow at least that time before checking the status of your application online at US Passport Services - Check Application Status |

To check the status of your application online you will need:
- Your last name, including suffixes without punctuations except the hyphen,
(for example: Jones III, Patton Jr., Jackson-Smith, Varela Garcia),
- Date of birth in the following format: MM/DD/YYYY, and
- The last four digits of your Social Security Number.

1-877-4USA-PPT (1-877-487-2778), TDD: 1-888-874-7793 Live Operator Service available 6:00 a.m. to midnight ET, M-F; and limited weekend hours to make emergency arrangements for travelers departing in 7 days or less.
Please note that representatives are available from 6:00 a.m. to 12 midnight, ET, Monday-Friday, except federal holidays. Representatives are also available for limited weekend hours for emergency situations in which travelers are leaving in 7 days or less.
