FMLA & OFLA

FMLA/OFLA secures the rights of employees to respond to the critical health needs for yourself and any qualified family member and to guarantee reinstatement to their former/equivalent position.
To be eligible you must have worked at least:
FMLA - 1260 hours in the preceding 12 months
OFLA - 25 hours per week in the preceding 180 days
Reasons to apply:
Serious health condition or injury to self
Serious health condition or injury to your spouse / same sex domestic partner
Serious health condition or injury to your parent / parent-in-law
Serious health condition or injury to your grandparents / grandparents-in-law
Serious health condition or injury to your child / step-child
Birth of a child / adoption
Protected time available
12 weeks in a rolling 12 month period or
480 intermittent hours in a rolling 12 month period
Accrued sick leave must be used first, then you can select any other accrued leave.
City of Salem will continue to your medical/dental/vision/group life whle you are on approved 12 week family leave
