Report an ADA / Section 504 Complaint
Reporting Discrimination or Unequal Treatment
If you believe you or any program beneficiary has experienced unequal treatment or discrimination in receiving benefits or services from the City due to a disability, you may file a report or complaint with the Title VI / Section 504 Coordinator.
Report Process
To file a complaint, please note the following:
- Timeframe: The report form must be filed within 180 days of the alleged discriminatory act or occurrence.
- How to File:
- Complete the ADA complaint form or download and complete a PDF version of the report form.
- Submit the completed form to the Title VI / Section 504 Coordinator.
Upon receipt of your complaint, you will receive acknowledgment within five business days, along with information about the steps that will be taken to address your complaint.
Informal Resolution
The City will make every effort to resolve your complaint at the lowest level possible. You may be asked to participate in an informal meeting with the Coordinator, City staff, or other affected individuals. If informal resolution is unsuccessful or if you prefer to proceed with a formal investigation, you may appeal the matter to the Coordinator, who will initiate a formal investigation.
Formal Investigation Process
Step 1: Department Response to Complaints
- The department mentioned in your report will have 15 days to provide a written response after being notified of the complaint.
- The Coordinator will then determine if further investigation is necessary. If so, an investigation will be conducted, which may include interviews with the complainant, relevant departments, program recipients, or other individuals with pertinent information.
Step 2: Written Report

Within 30 days of receiving the complaint, the Coordinator will prepare a written investigative report that includes:
- A description of the incident
- Identification of individuals interviewed
- Investigation findings
- Recommendations for resolution

The report will be reviewed and finalized by the ADA Coordinator and sent to the City Manager for appropriate action.

Once the investigative report is complete and action is determined, you and the relevant department will receive:
- A copy of the investigative report
- A statement of the appropriate action taken
- Notification of your appeal rights
Step 3: Meeting
- Within 15 days of sending the written report to you and the department, the Coordinator will offer to meet with you to discuss the determination of appropriate action, along with the findings and conclusions from the investigative report.
- If you have new facts that were not previously considered and could not have been reasonably discovered during the investigation, you may request changes to the appropriate action statement.
Other Report Options
If you are not satisfied with the results of the investigation or the resolution of your complaint, you may file a complaint directly with the United States Department of Justice or another appropriate state or federal agency. Using the City’s procedure is not a prerequisite for pursuing other remedies.
Disclaimer
The resolution of any specific complaint requires careful consideration of various circumstances, such as the nature of the disability, access to services, essential eligibility requirements for participation, health and safety of others, and the extent to which an accommodation would fundamentally alter the program, service, or facility or impose an undue hardship on the City. Therefore, the resolution of one complaint does not set a precedent for future cases or obligate the City to respond in a similar manner to other complaints.
